Why is my mother paranoid and thinks people are stealing?

She refused to go back to a neurologist my brother and I took her to, because she saw where a secretary had written “dementia” in the margin of her lab result. She has always been a Jekyll/Hyde personality, but now she is very paranoid and delusional, it’s always directed against the woman closest to her.
